Staff Shout-out: Our staff shout out form is back! I want to provide a space for parents to show their gratitude for our hardworking staff. If you would like to give a shout out to a staff member, please fill out the following form. We will share all submissions with our staff. Parent to Staff Shout out Form
Title IX Student Training: On January 24, 2013, Gov. Pat Quinn signed a law, Erin’s Law, into effect for all Illinois schools. Erin’s Law requires schools to provide age-appropriate sexual abuse prevention education for all students on an annual basis. The Office of Student Protections and Title IX has collaborated across a number of departments to create developmentally appropriate sessions that will allow students to identify abuse and empower them to report any wrong doing that may have occurred. The sessions have been broken down by grade, and are all around 30-45 minutes. All training will be conducted in homeroom by November 22nd, 2024.
This is an important learning opportunity for students. However, a parent or guardian may opt their child out of the training if they do not wish for them to participate. The opt-out form will be good for the current school year only. If you choose to opt your child out, please complete this google form. If you wish to teach your child this material yourself, you can access the videos to the sessions on the OSP website at www.cps.edu/osp.
YOU DO NOT NEED TO RESPOND TO THIS FORM UNLESS YOU ARE OPTING-OUT YOUR CHILD FROM THIS PRESENTATION.
Level I & Level II Volunteer Application Process: All individuals who are interested in volunteering (includes chaperoning field trips) must be approved by CPS. Depending on the level of volunteering you plan to do, you will be asked to complete an application for either a “Level I” or “Level II” volunteer.
Steps to follow: (Please note: It is a CPS requirement that you fall into one of these two levels to become a volunteer)
Step 1: Complete THIS volunteer form for CPS This form is necessary for CPS to verify you have students in the system and that they attend the school you are requesting to volunteer at.
Step 2: Bring a government issued ID to the main office for verification. If you do not do this step, the process will not move forward.
For Level II, the process ends here.
For Level 1, you must also do the following:
Step 3: After you submit your ID, you will receive an email from CPS which will contain links and information for fingerprinting, background check and TB test. For fingerprinting, please fill out this form and take it to any Accurate Biometrics location. For the TB test, use this form. These tests can be completed at MercyWorks locations. Please note, there is a $20 TB test fee.
Step 4: Email copies of both the fingerprints and TB test to [email protected]. The sooner they receive these forms, the faster your application can be processed. Please note, fingerprints can take 2-5 weeks to process.

Halloween Parade
On Thursday, October 31st, we will have our annual Halloween Parade which will begin on the turf around 2:15pm for students in Grades K-4. Join us to see all of our scary and creative costumes this year! We ask that adults stand on the middle of the turf to allow the parade to proceed around the turf without interference.
All students in grades K-8, who choose to participate, are allowed to wear a school appropriate costume, that will not be a disruption to instruction. All students can wear the costume to school. Keep in mind the younger students must be able to manage the costume by themselves. No guns, swords, or other weapon replicas. Masks and face paint are also not allowed.
